Pre-screening Questions

Daily self-certification screenings are no longer required for students, faculty, and staff. By sending a student to school or coming to work as an employee, individuals are automatically acknowledging that they have answered "no" to the following questions posted below. Please note that if transmission rates continue to increase, we will reimplement daily screenings. Students that are sent to the Nurse's Office during the school day for symptoms in line with sickness, may be sent home based on evaluation by the Nurse's Office/Administration.

By sending your student(s) to school, you have answered no to the following questions:

  1. Have you traveled internationally within the past 3-5 days and not submitted a negative COVID test to Catherine Cook?
  2. Have you had a body temperature higher than 100.4 degrees, or have you used fever-reducing medication within 24 hours to treat a fever? 
  3. Have you experienced any GI symptoms such as nausea, vomiting, diarrhea, or loss of appetite in the past 48 hours? 
  4. Are you experiencing shortness of breath or difficulty breathing? Are you or any member of your household awaiting a COVID-19 test result due to close contact with an individual diagnosed with COVID-19?
  5. Do you have a new cough that you cannot attribute to another health condition (example: allergies)? 
  6. In the past 10 days, have you been in close contact with anyone diagnosed with COVID-19?
  7. In the past 10 days, have you or any member of your household tested positive for COVID-19?
  8. Are you or any member of your household awaiting a COVID-19 test result due to close contact with an individual diagnosed with COVID-19?
