Health Requirements

We will be continuing to use Magnus Health to collect and store student health forms and medical information for the 2017-18 school year. Please review the following information to decide if any forms or information is required.

Requirements For All | 2016-2017

Vital Health Record in Magnus

  • You will need to review each section to update where necessary and save. Your information entered last year will be visible. If no changes are needed, or after changes are made, you must save each section to complete the requirement and submit for approval.


Required Forms by the State of Illinois

  • Physical Health Exam/Immunization Form: If your child is entering PS, SK or 6th Grade ONLY, or your child is a new student to Catherine Cook this year (all grades for new students), please have your doctor complete and submit this form by August 15, 2017. This form should be completed from an exam within the last 12 months. The physical health exam form is NOT VALID if the Health History portion is not completed and signed by a parent/guardian. Also, lead results must be included on all health forms for students 6 years of age and younger.
  • Dental Exam Form: If your child is entering SK, 2nd Grade, or 6th Grade, please have your dentist complete and submit this form by August 15, 2017. If your child is new to Catherine Cook this year (grades SK-8th Grade ONLY for new students), please submit this form by August 15, 2017. This form should be completed from an exam within the last 12 months.
  • Vision Exam Form: If your child is entering SK, please have your optometrist complete and submit this form by August 15, 2017. This form should be completed from an exam within the last 12 months. If your child is new to Catherine Cook this year (grades SK-8 Grade ONLY for new students), please submit this form by August 15, 2017.


Annual Health Forms Required by Catherine Cook (due by August 15, 2017)

  • Food Allergy Action Plan: In order to protect the safety of children who have food allergies, we require that ALL students with allergies requiring emergency medications (Benadryl/Epi Pen/Inhaler) complete this form as a standard emergency action plan. It must be signed by you and your child’s doctor. Please also review the Catherine Cook Allergy Guidelines for parent(s) and guardian(s) responsibility regarding notification, communication, medication, and education.
  • Asthma Action Plan: If your child has asthma and receives emergency medication such as an inhaler, this form is required as a standard emergency plan.


Applicable Annual Health Forms by Catherine Cook
(Found in Magnus Health via the Family Login and due by August 15, 2017)

  • Prescription Authorization Form: If your child needs any post-illness (ex: strep throat/pink eye/oral steroids/inhaler) prescription medication or any prescription maintenance medicine (ex: daily ADD/ADHD medication) administered at school, please have your child’s doctor complete this form before the medication can be administered at school. All prescription medication must be in their original prescription bottle labeled clearly with the name of the student, medication, dosage, and time to administer.
  • Non-Prescription Authorization Form: If your child is prone to headaches, minor aches and pains, allergies, coughs, or any other malady that can be treated with an over-the-counter medicine such as Motrin, Tylenol, Benadryl, or cough drops, we ask that this form be completed online in Magnus. Non-prescription medication may not be given unless this form is completed and signed by a parent or guardian.
  • Asthma Self-Administer Form: If your child prefers to carry their own inhaler, this form is needed. Any student with asthma is required to keep an inhaler in the School Nurse’s office in addition to this.

Please note the following requirements for your child for the upcoming 2017-2018 school year at Catherine Cook School. These medical forms are required and due by August 15, 2017. To insure and protect your child’s privacy, parents are responsible for uploading completed requirements directly to Magnus Health via the Family Login. Magnus is an online secure medical records system used by Catherine Cook. Please do not mail in or drop off health forms. Catherine Cook does not need hard copies at the school, so you may keep those for your personal records. According to the Illinois State Board of Education, students who are noncompliant with their physical examinations and immunizations by October 15 will be excluded from school.

Preschool Health Requirements | 2017-2018

Preschool Students for 2017-2018 School Year

PHYSICAL EXAMINATION

1. PHYSICALS completed within a year prior to entry and signed by a physician.

2. HEALTH HISTORY SECTION needs to be completed and signed by the parent or legal guardian. Forms without completed health history are not considered complete.

3. LEAD ASSESSMENT documentation, indicating the health care provider has addressed lead poisoning, is required.

IMMUNIZATIONS

1. POLIO: Three or more doses of polio (IPV or OPV), at the appropriate intervals.

2. DIPTHERIA, TETANUS, PERTUSSIS (DTaP): Four or more doses of DTaP, at appropriate intervals.

3. MMR (MEASLES, MUMPS, RUBELLA): one dose of vaccine, the first being on or after 12 months of age.

4. VARICELLA (chicken pox): one dose.

5. HEPATITIS B Series: 3 doses at the appropriate intervals.

6. HIB: Dosing at appropriate intervals.

7. PNEUMOCOCCAL: Proof of immunization that complies with the pneumococcal vaccination schedule.

In addition a Food Allergy Action Plan (completed by physician) is required for all students with food allergies and an Asthma Action Plan (completed by physician) is required for all students with asthma.

Kindergarten Health Requirements | 2017-2018

Kindergarten Students for 2017-2018 School Year

PHYSICAL EXAMINATION

Second Grade Students for 2017-2018 School Year

DENTAL EXAMINATION

Dental examinations dated on or after January 1, 2017.

EMERGENCY CARE PLAN

If your child has a medical condition that requires medication administration, please submit a Medication Authorization Form (completed by physician). In addition, a Food Allergy Action Plan (completed by physician) is required for all students with food allergies and an Asthma Action Plan (completed by physician) is required for all students with asthma.

6th Grade Health Requirements | 2017-2018

Sixth Grade Students for 2017-2018 School Year

PHYSICAL EXAMINATION

1. PHYSICALS completed within a year prior to entry and signed by a physician.

2. HEALTH HISTORY SECTION needs to be completed and signed by the parent or legal guardian. Forms without completed health history are not considered complete.

IMMUNIZATIONS

1. POLIO: Four or more doses of polio (IPV or OPV), at the appropriate intervals.

2. DIPTHERIA, TETANUS, PERTUSSIS (DTaP): Four or more doses of DTaP, at appropriate intervals.

3. TETANUS, DIPHTHERIA, ACCELUAR PERTUSSIS (Tdap): one dose.

4. MMR (MEASLES, MUMPS, RUBELLA): two doses of vaccine, the first being on or after 12 months of age.

5. VARICELLA (chicken pox): Require two doses.

6. HEPATITIS B Series: 3 doses at the appropriate intervals.

7. Meningococcal: 1st dose received on or after the 11th birthday.

DENTAL EXAMINATION

1. Dental examinations dated on or after January 1, 2017.
