Catherine Cook School follows all compliance guidelines set forth by the State of Illinois. Catherine Cook requires all parents and guardians to review and complete the appropriate forms by their due dates prior to their student(s) beginning classes.

Please refer to the following link to determine what health requirements are needed for your student for the current school year. Magnus is an online secure medical records system used by Catherine Cook. All health forms are available in Magnus and it is the parent’s responsibility to upload them electronically to the system. No health forms are to be sent or given to the school. Parents and guardians, please go to the Cook Portal and select Magnus Health under the Resources tab.
