Catherine Cook School follows all compliance guidelines set forth by the State of Illinois. Catherine Cook requires all parents and guardians to review and complete the appropriate forms by their due dates prior to their student(s) beginning classes.
Please refer to the following link to determine what health requirements are needed for your student for the current school year. Magnus is an online secure medical records system used by Catherine Cook. All health forms are available in Magnus and it is the parent’s responsibility to upload them electronically to the system. No health forms are to be sent or given to the school. Parents and guardians, please go to the Cook Portal and select Magnus Health under the Resources tab.

Changes to the Health Office
There is an isolation room, separate from the health office, for any individuals experiencing signs of illness. To limit movement, certain complaints will be handled safely in the classroom. Classrooms are stocked with basic first aid supplies, including Band-Aids, gloves, antiseptic, and ice packs.
When a positive case is reported through Covid@ccookschool.org the individual or individual’s parent/guardian will be contacted by the Pandemic Coordinator or School Nurse. The Pandemic Coordinator will work with the individual to identify anyone in the Cook community who they have been in close contact with beginning 48 hours prior to symptom onset or the date of the positive test. The Pandemic Coordinator will report all confirmed positive COVID-19 cases to CDPH and, in collaboration with CDPH, will verify close contacts.

The school nurse office hours are from 8 am - 4:00 pm Monday through Friday. Emails received after hours or on the weekend will be returned the following school day. If you require immediate assistance related to COVID-19, please email our COVID-19 Response Team at Covid@ccookschool.org or Pandemic Coordinator at Lskinner@ccookschool.org.
