Health and Wellness
The School Nurse, Jennifer Naponelli, R.N., B.S.N., is responsible for our health compliance with the State of Illinois. Please review and complete the appropriate forms by the appropriate date in order to begin school at CCS.
Physical Health Exam Forms, Immunizations Forms, Dental Exam Forms, and Vision Exam Forms are only needed for students entering specific grades. Please carefully review this file to make sure your child is up to date each year and that you are in compliance with the state for sending your child to the school.
Physical Exam/Immunization Form: If your child is entering PS, SK, or Grade 6 ONLY, or your child is new to CCS this year (all grades), please have your doctor complete and return this form by August 15, 2010. Please remember to fill out and sign the Health History area.
Dental Exam Form: If you child is entering SK, Grade 2, or Grade 6 or your child is new to CCS this year (grades SK and above ONLY), please have your dentist complete and return this form by March 1, 2011.
Vision Exam Form: If your child is entering SK or your child is new to CCS this year (grades SK and above ONLY), please have your optometrist complete and return this form by October 1, 2010.
Students in all other grades not listed above do NOT have any other State Health Requirements.
In addition, all students will have Hearing and Vision Screening completed the second week of September here at The Catherine Cook School. This will be an additional fee of $5 per student per screening, billed accordingly through the Business Office after the exam is ccompleted. If your child has had an eye exam in the last 12 months and you would prefer to defer on this screening or if you do not wish for your child to recieve either of these screenings for any other reason, please email Jennifer Naponelli. Students in SK will not recieve a vision screening as an Eye Exam is mandatory instead for their grade level.
2010-2011 Medication Authorization Policy
If your child needs any prescription, non-prescription, or emergency care for any illnesses, please review our updated policy regarding medication administration. All medication must be clearly labeled when you return the completed form to the school. The school will not be able to provide any medication without the proper authorization form on file. If you prefer that we do not administer any medication or if your child does not have any emergency conditions, these forms are not required. All of these forms need to be re-submitted each year on an annual basis.
- 2010-2011 Prescription Authorization Form: If you child needs any post-illness prescription medication or any prescriptio maintenance medicine administered at school, please have your child's doctor complete this form and return it to the school at your earliest convenience.
- 2010-2011 Non-Prescription Authorization Form: If your child is prone to headaches, minor aches and pains, allergies, cough drops, cough medicine, or any other malady that can be treated with an over the counter medicine, we ask that this form be completed. A doctor's signature is not required.
- 2010-2011 Asthma Self-Administered Medication Authorization Form: If your child is able to treat his or her asthma on his or her own with an inhaler, please complete this form at your earliest convenience.
- 2010-2011 Food Allergy Action Plan: In order to protect the safety of children who have food allergies, we ask that ALL students with food allergies complete this form as an a standard action plan. It must be signed by you and your child's doctor.
